When you become a patient of this practice, you provide consent for Omoi Health to access and use your personal information so we can provide you with the best possible health care. Only staff who need to see your personal information will have access to it. If we need to use your information for anything else, we will seek consent from you to do this. The main purpose for collecting, using, holding, and sharing your personal information is to thoroughly assess and manage your health needs. medical information including presenting complaint, medical history, current and previous medications, known allergic reactions, adverse events, immunisations, past and family history, relevant lifestyle history, risk factors, findings from pathology tests or other investigations.
your preferences and interests including future topics, products or services that may interest you
data gathered by Google Analytics such as pages browsed to improve the usability and appeal of the website.
Sensitive information that is collected such as contact details and medical history is so we can provide you with our services and to ensure we are providing you with the best possible care. This information is collected via emails, intake forms and on the practice management software Simple Clinic, and is securely stored on a password protected computer.

You have the right to deal with us anonymously or under a pseudonym unless it is impracticable for us to do so or unless we are required or authorised by law to only deal with identified individuals.

when you make your first appointment we will collect your personal and demographic information via your registration.

We will not share your personal information with anyone outside Australia (unless under exceptional circumstances that are permitted by law) without your consent.

Your personal information is stored on a password protected computer and may be stored securely online, or in the cloud through Simple Clinic (Practice Management Software). Only Omoi Health employees have access to this information and we are committed to securely storing and handling your personal information.

How can you access and correct your personal information?
You have the right to request access to, and correction of, your personal information. This practice acknowledges patients may request access to their medical records. We require you to put this request in writing and email this to us and we will respond within a reasonable time (30 days).
Unless we are subject to a confidentiality obligation or some other restriction on giving access to the information which permits us to refuse you access under the Privacy Act, and we believe there is a valid reason for doing so, we will endeavour to make your information available you within 30 days. We will take reasonable steps to correct your personal information where the information is not accurate or up to date. From time to time, we will ask you to verify that your personal information is correct and current. You may also request that we correct or update your information, and you should make such requests in writing to hello@omoi.com.au.
How can you lodge a privacy-related complaint, and how will the complaint be handled at this practice?
We take complaints and concerns regarding privacy seriously. You should express any privacy concerns you may have in writing. We will then attempt to resolve it in accordance with our resolution procedure within 30 days.
